3.本项目的特定资格要求：
合同包1(万安禅院石窟景区灾后修复工程)特定资格要求如下:
（1）具有独立承担民事责任能力的法人、其他组织或自然人，统一社会信用代码营业执照（事业单位提供事业单位法人证书，自然人提供本人身份证）合法有效；（2）投标人需具有法定代表人授权书（附法人、被委托人身份证复印件）；（3）财务状况报告：提供开标前近三年内(2019年、2020年、2021年）任意一年的财务会计报告（至少包括审计报告、资产负债表和利润表。成立时间不足一年的需提交投标文件截止时间任意时段的资产负债表）或其基本存款账户开户银行出具的资信证明（或）基本存款账户信息或政府采购信用担保机构出具的投标担保函；（4）税收缴纳证明：开标前6个月内连续三个月已缴纳的纳税证明或完税证明，依法免税的供应商应提供相关文件证明；？（5）社会保障资金缴纳证明：供应商需提供开标前6个月内连续三个月已缴存的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的供应商应提供相关文件证明；（6）投标人应具备古建筑工程专业承包二级及以上资质，具有建筑行政主管部门颁发的安全生产许可证，拟派本工程的项目经理必须为注册于本单位的建筑工程专业二级及以上建造师，且具有有效的安全生产考核合格证书（B证），无在建项目；（7）投标人需出具参加政府活动前三年内，在经营活动中无重大违法记录的书面声明；（8）投标人不得为“信用中国”网站（www.creditchina.gov.cn）中列入失信被执行？人？或？重？大？税？收？违？法？案？件？当？事？人？名？单？的？供？应？商？，？不得为中？国？政？府？采？购？网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的供应商；（9）单位负责人为同一人或者存在控股、管理关系的不同供应商，不得同时参加本招标项目投标；（10）本项目不接受联合体投标。
